Important Logistics—PLEASE READ
RECENT/CURRENT APPOINTMENT
- If you had an appointment in last 3 months or have an appointment scheduled through November 7th 2025, you do not need to do anything. You will receive a call for your telehealth visit on the scheduled day and time.
- If you recently had labs or imaging, you do not need to do anything. We will reach out to you with your results and to create a follow up plan
- If you have an acute problem but DO NOT have an appointment scheduled, use the schedule an appointment link on our website to make an appointment request. We will reach out to you and make every effort to meet your needs in the next 4 weeks.
MEDICAL RECORDS REQUEST
- Patient medical records will be transferred securely and at no cost, upon request.
- Requests and correspondence can be directed to our encrypted email at: info.centralphoenixobgyn@proton.me with the subject line RECORDS REQUEST. You must use this subject line to appropriately route your email and avoid delays. Please include identifying information (patient name, DOB, and receiving provider details). Please allow 14 business days to fulfill your request. All records will be sent electronically. We do not have the capacity to send hard copy records before November 7th. (We also do not recommend sending your private health information in the mail.)
- We encourage you to submit any records requests before November 7th, 2025, if possible, though all requests will still be honored after that date.
- BILLING ISSUES
If you have an outstanding balance, or receive an invoice you have several options for payment:
- Use the link in your text invoice to submit your payment electronically
- If you want to pay by check, you can send it to us at our new contact address below
- If you want to pay by phone, send us an email to centralphoenixobgyn@proton.me with the subject line BILLING QUESTION. You must use this subject line to appropriately route your email and avoid delays
