Appointment Scheduling and screening

Before your appointment, you will need to complete and submit any necessary forms electronically via our patient portal.  You will receive an email/text prior to your appointment with a link to our patient portal where you can find detailed instructions on the forms you will need. Your appointment may  need to be rescheduled if forms are not completed 48 hours in advance of our appointment date. 

Visitor Policy

Visitors are welcome at your live visits. To optimize time and attention during your visit, we continue to recommend you limit the number of visitors you bring with you and. As always, you can use phone or video  chat to include loved ones in your clinical session. 

Masks

For the protection of patients and staff, you may be asked to wear a face mask to cover your mouth and nose while in the office suite. If you do not have an effective face covering, one will be provided for you. We will help you with rescheduling your appointment if health conditions prevent you from wearing a face covering when needed. For your protection, our staff may also be wearing a face covering during interactions with you.
